Italy and Bulgaria both avoid PLAB. The difference is the money: Italian public tuition can be a fraction of the Bulgarian fee. Recognition without PLAB is subject to a government review in 2028.

What is the IMAT, and how is it marked?
Sixty multiple-choice questions in 100 minutes. A correct answer scores +1.5, a wrong one −0.4, and a question left blank scores zero. The maximum is 90 points.
Because wrong answers cost marks, knowing when not to answer is part of the exam.
How many universities can my child apply to?
One. A non-EU applicant resident outside Italy names a single university and competes only in that university’s own ranking, not a national one.
Families living in the UK are in this category, whatever their nationality. Choosing that one university well is the most consequential decision in the whole application, and it is the part we spend most time on.
Is there a minimum score?
Not for non-EU candidates resident abroad. Any score above zero is eligible; only a completely blank paper is excluded. The 20-point threshold that is often quoted applies to EU candidates and those treated as equivalent, not to this group.
Eligible is not the same as admitted. Places are filled in rank order.
Can my child work as a doctor in the UK afterwards?
An Italian medical degree is a relevant European qualification, and the General Medical Council does not require PLAB for it.
Two things to hold alongside that. Recognition without PLAB is subject to a government review in 2028. And UK and Irish graduates are offered Foundation Programme places ahead of everyone else, which affects the first job after graduation rather than registration itself.
